Grant Request Procedures 
All Grant Requests Are By Invitiation Only
If you are invited to submit a grant request, you must be a 501(c)(3) organization to qualify. There is not an application form to complete; you will need to prepare a grant request for funding that addresses the required information. Our grant request procedures are listed below.
Overall criteria and instructions:

Your grant request must contain all required information and adhere to the stated format and guidelines. |

Your grant request must fall within the guidelines for what we fund. |

Only one unbound copy of your grant request is needed. |
 |
It should not exceed the maximum page limits where indicated. |
 |
All information submitted must be in English. |
 |
Please do not send brochures, catalogs, photos, pamphlets, tapes, DVD’s or other supplemental information. If we need additional information to facilitate the grant making process, we will let you know. If you include any supplemental materials or items, the entire grant request will be returned. |

You must adhere to the submission deadline as stated in your invitation letter. We will not consider any request received after the stated deadline. |

What your grant request must provide:

Cover letter on organization’s letterhead with contact information |
 |
Brief history and description of the organization (1 page maximum) |
 |
Project description of the request (2 pages maximum)
Project description to include:
- The specific dollar amount requested
- How and when the project has been or will be evaluated. Please include outcome evaluations

List of officers and directors of the organization and those responsible for the project |

Minutes of your most recent two (2) Board meetings |

Financial Reports:
- Project Budget
- A schedule of the Year-to-Date funding received for the project by source
- A schedule of your organization’s Major Sources of Revenue for the current Year-to-Date and for the prior Fiscal Year by category - Cash Donations, In-Kind Support, and Earned Revenues (1 page maximum):
For each category, please include:
- For Cash Donations and In-Kind Support, please provide:
The total dollar amount by type of donor: Individuals, Foundations (names of the entities and dollar amounts), Corporations (names of the entities and dollar amounts), Governmental Agencies (local, state, federal), United Way, Civic and Religious Organizations, and Other.
- For Earned Revenues, please provide a detailed breakdown by significant sources, such as special events, program fees, investment income, etc.
- Current Year Annual Organization Budget approved by the Board
- A schedule of the Current Year-To-Date Revenues and Expenses vs. the Current Year-To-Date Budgeted Revenues and Expenses
- A schedule of the Prior Year Revenues and Expenses vs. the Prior Year Budgeted Revenues and Expenses
- Statement of Financial Position for the Current Year (as of the most recent Month End Date) and for the Prior Year (as of the Fiscal Year End Date)
(Balance Sheet)
- Copy of most recent IRS tax determination letter (must be dated within the last 10 years)
- Copy of most recent filed IRS Form 990. If a Form 990 is not filed by your organization, please submit an explanation as to the reasons why.

Instructions to submit financial reports:
Please label each financial report with its corresponding capital letter. If a financial report is not applicable or available, please explain. Address it briefly, but do not dismiss it.
Time Requirements
The Embrey Family Foundation Board of Directors meets twice per year – in late Spring and late Fall – to review By Invitation Only grant requests. Grant requests will be awarded to those applicants for funding by the Board at one of the Foundation’s semi-annual meetings.
Grant requests from invited grantees will be due by the submission deadline as stated in your invitation letter.
Your grant request must be complete and adhere to our guidelines and criteria to be considered. Failure to meet the grant request deadline or failure to provide a complete grant request with all required and applicable documentation will automatically eliminate the applicant from consideration.
According to our Guidelines, if your grant request is approved, we require that a report be sent to the Foundation outlining the specific outcomes of the program we helped to fund. A letter will be sent to your organization outlining the report requirements and deadlines.
